Name That AIDS Program: PEPFAR II Naming Contest

The U.S. President's Emergency Plan for AIDS Relief (PEPFAR) - a 5-year, $15 billion initiative to support the prevention, treatment, and care of HIV/AIDS in some of the worst-affected countries - expires at the end of 2008, and Congress is currently working on legislation to extend the program for another 5 years. President Bush has called for $30 billion for the next phase of the program, with many key congressional leaders and presidential candidates pushing for $50 billion.

With many important issues to be resolved about the next phase of PEPFAR, one issue that is not often discussed is the program's name. CGD's HIV/AIDS Monitor invites you to offer your choice for the naming of PEPFAR II.

Why?
Much has changed since the U.S. Leadership Act established PEPFAR in 2003. At the time, HIV was spreading fast, donor money for HIV/AIDS was scarce, and an HIV diagnosis was a virtual death sentence in the developing world. Since then, due to a significant increase in international funding led by PEPFAR and the Global Fund to Fight AIDS, Tuberculosis and Malaria, HIV/AIDS programs have been greatly expanded in the most affected countries, and hundreds of thousands now have access to life-saving ARV drugs. These developments require an updated strategy and a name for the U.S. AIDS program that reflects it.

For one, changes in the global AIDS landscape require policymakers to think more seriously about how PEPFAR can transform itself from an "emergency" plan , where long-term planning can sometimes take a backseat to the need to roll-out programs as quickly as possible, to a more sustainable strategy based on building the capacity of host countries to coordinate and implement their own AIDS responses. A program with "emergency" in the title does not accurately reflect the successes of the last 5 years or the new challenges we face in sustaining and building on this success in the future.

The name of PEPFAR's next phase should also reflect the fact that it is a program with widespread, bipartisan support. While much credit goes to President Bush for mobilizing U.S funding for PEPFAR I, a "President's" program for PEPFAR II runs the risk of being associated with President Bush only, potentially reducing support from Democratic congressional leaders and/or the next president. Changing PEPFAR's name to reflect its wide base of support would make PEPFAR more than just a Bush program and encourage a sense of ownership and shared responsibility on the part of the U.S. government to continue its commitment to global AIDS programs. As our colleague Mead Over has emphasized, this has implications for the U.S. pledge to keep patients on ARV treatment - what he has termed the "AIDS entitlement." A Call for Ideas
As reauthorization moves forward, we invite you to offer your own ideas for what the next phase of PEPFAR should be called. What name would capture the need to build a sustainable program over the next 5 years? What name would reflect the U.S.' long-term commitment to battling the global HIV/AIDS pandemic? What name would acknowledge the connection of HIV/AIDS programs to the broader global health and development agendas? The choice is yours.

I concur with the suggestin that the new name for PEPFAR II should refelect the bipartisan support to the plan that was instituted as an emergency response to a worldwide crisis. I also agree that the name should encourgage other capable donor governments that have yet initiated any assistance to the most affected countries of the world to become integrated to a world movement to confront the AIDS pandemic, by core-streaming AIDS assisatance into their bilateral and multilateral relations with countries of some defined threshold of infection rates. Finally, while President Bush will forever me remembered as the leader who inspired American leadership in the fight against AIDS, the new name should inspire successive U.S. governments to forge on with the leadership the world needs in this fight. Pursuant to these views, I suggest that the following new name for PEPFAR II:
"LEADERSHIP IN GLOBAL MOBILIZATION FOR AIDS RELIEF" (LIGMAR) in focus countries

HIV/AIDS Assistance from the American People (HAAAP).

Simply put: Through PEPFAR in its current state, and in the reauthorization to perpetuate the Plan, average American taxpayers reach across the globe to assist in addressing the pandemic. It might be nice to just be clear about that. Clarity, brevity, long-shelf life, easy to dance to...

One proposal and the variation I thought about:

USLEAD - United States Legacy against AiDs

The idea was to bind LEADership with Legacy in order to highlight the importance in terms of time of the plan: now and future. It came after

USLEAP - United States Legacy against Aids Pandemic

where capitals match but the clarity of the time simile is blurred, and changing it for Leadership remains an option.

"US Program to Sustain Progress in HIV/AIDS Eradication". If we must use an acronym or catchy title then we could call it "Project Sustain Progress - PSP". The use of the longer title is preferred because it provides all the key words with an emphasis on the long term objective of eradicating HIV/AIDS from the face of the earth.

US Fund for Sustainable Prevention and Control of HIV and AIDS(USFSPC-HIVand AIDS)

- The fund comes from US, therefore, it is necessary to indicate the source of the fund.
-HIV and AIDS cannot be dealt in an emergency manner, therefore suatainable prevention and control activities need be in place.
Moreover sutainbility means a multisectoral response to the prevention and control of HIV and AIDS. Besides, sutainability is underpinned by commitment of stakeholders to use of the fund effectively to halt the spread of HIV. The fund would require stakholders to use it wisely and effectively by designing cost effective approaches during program implimentation. The fund can lend itself to be used in a harmonized and cordinated manner along with local resources and systems of recepient countries, and other donor resources as well.This would help in the creation of the necesary capcity in a genric manner in the recepient countries so that they could assume full responsibility and leadership in dealing with HIV and AIDS.
I think the fund can be used in a manner that it could play a leading role in bringing HIV to a level of insignificance, if not total irradication

I propose that PEPFAR be renamed to APFAR - the "American Plan for AIDS Relief".

Inspired by a suggested made to me by Jim Cashel, I suggested this at a public meeting on 21 July 2005 on "The Long-Term Financing Implications of the HIV/AIDS Pandemic", organized by the Kaiser Family Foundation. On page 76 of the transcript, available at www.kaisernetwork.org/ health_cast/uploaded_files/ 1622-%20072105_kff_csis_transcript.pdf, you'll find the following:

Hi. I’m Bernard Rivers. I’m from a NGO called Aidspan which serves as a watchdog of the Global Fund.

A year ago, at the Bangkok International AIDS Conference, there was a problem … because there was a sense, particularly amongst the activists, that PEPFAR could do no right and the Global Fund could do no wrong. They were wrong on both fronts.

I think part of the problem on the PEPFAR front is a problem with branding. Everyone here has been using the term PEPFAR except the folks from PEPFAR, so they don’t even know quite what their name is, or they don’t know when to use that name PEPFAR.

PEPFAR stands for the "President’s Emergency Plan For AIDS Relief". I have two problems with that; the first is the use of the word "emergency" and second is the use of the word "president".

It’s not, I think, an emergency plan -- it’s not a tsunami response, it’s not something that’s going to be wrapped up in a year or two once the emergency is over, because the emergency is not going to be over, so I think the word "emergency" should be dropped. You can respond rapidly, as in an emergency, but I think the word "emergency" doesn’t really belong there.

The second is the use of the word "president". Mark Dybul himself said today, "President Bush’s emergency plan". I don’t think that’s the title of the organization. If President Bush had not been reelected in the last [2004] election, would that plan have stopped? I certainly hope not. I hope that the next president will continue this plan, so I don’t think it’s President Bush’s emergency plan. Yes, good for him. He started it, he initiated it and he deserves credit for that. But I think a better name would be APFAR, the "American Plan for AIDS Relief". We’ve heard a lot about the American people, yes good for them, they are financing that through their taxes. I humbly suggest a new name, APFAR. Thank you.

I propose ENABLE - Empowering National Approaches Based on Leadership and Evidence.

Whether or not the Office of the Global AIDS Coordinator adopts a new name for PEPFAR, I hope that the second phase will emphasise sustainability by promoting national ownership through ongoing high-level policy dialogue (leadership). Additionally, I also hope that research and innovation will drive the interventions and investments in PEPFAR 2 (evidence).

I would suggest USAGRAPP -USA global response to AIDS pademic plan
PEPFAR has done alot in our country Kenya in dealing with the pademic.It is worth while and therefore need to continue. The supporting agencies need to think also how to make the programme sustainable.In developing world we have many health problems which need support especially promotive and preventive health services and their systems.

There is already a Global Fund for Aids, TB and Malaria. If the USA really wants to "restore its intellecutal leadership in development" why not put funds through this, and make it more effective, rather than maintain a parallel bilateral structure?

Below are multiple suggestions. Some of my entries reflect AIDS-focused concepts, while others reflect a need to twin general healthcare system capacity building efforts with AIDS-centered responses. Where possible, I tried to reflect sustainability concepts.

PEPFar 2p: Particularly Expensive Program FAR from Partner Priorities

Rwanda is one of fifteen PEPFAR "focus countries." It also is inundated funds for HIV/AIDS from GFATM. But the priority HIV/AIDS receives does not square with Rwanda’s own development priorities.

Indeed, Rwanda spends about $48 million per year for HIV/AIDS matters—but HIV/AIDS affects only three percent of the population. (Source: Rapporteur’s Report. Follow-on Meeting to the Post-High-Level for on the Health MDGs. Tunis, Tunisia, June 12-13, 2006.) On the other hand, Rwanda only spends $ 1 million per year on maternal and child health programmes. In terms of dollars spent per life saved, this is grossly inefficient, and it results from a lack of donor responsiveness to partner priorities.

I think that the original acronym of PEPFAR should be maintained but the words within the acronym changed so as to erase the problematic words "emergency" and "President". Here are a few suggestions along those lines:

Program to Enhance and Promote Further AIDS Relief

Program to Empower and Partner For AIDS Relief

Partnership to Encourage Progress in Fighting AIDS Rapidly

Program to Encourage/Empower Partnerships Focused on AIDS Relief
